This form is used to indicate the satisfactory completion of the student's final requirement(s). This form must be submitted with committee signatures or program director's signature to the associate director of graduate student services in the Office of Advising and Student Services no later than the last day of classes for the degree conferral term. Failure to do so may result in the denial of degree conferral.
