Academic Probation Self-Assessment

The College of Science and Health believes self-reflection can be key to helping students understand how they can be academically successful.   Students on academic probation or those struggling to achieve academic success are encouraged to complete the academic self assessment and share it with their advisor during an advising appointment.  

The self-assessment prompts students to identify obstacles or challenges that affect their academic performance as well as their academic strengths and skills.  The second portion of the self-assessment prompts students to create goals and actionable steps they can take to help achieve those goals.  Advisors can help students identify resources available throughout campus and offer support throughout the quarter.
