This form is to be used by students who are earning clinincal hours at external clinics or sites.

• Clinical Observation Hours Verification Form

This form is to be filled out by students inthe Speech Language Pathology program to attest to their promise to maintain confidentiatlity with respect to their interactions with clients, peers, and faculty and staff.

• Confidentiality and Privacy Oath

This form must be filled out whenever your are photographing an individual who is not an employee of DePaul University and you intend for said photograph to be used for official university business as specified in the document.

• Photo Release Form

This application is intended only for graduate College of Health and Science students. Please read and understand the University’s policies on taking courses on a pass/fail or audit basis.  ​Submit completed pass/fail/audit form to CSHAdvising@depaul.edu.

If student leaders want to plan and carry out an event, they need to fill out the SLP Student Leadership/Event Approval Form and have it reviewed by their faculty sponsor. The sponsor will then present the request to the program director for final approval. Students must give as much information as possible to increase their chances of getting approval for their event.

This form is to be filled out by students in the Speech Language Pathology program to opt-out an assigned student placement starting in their second year of winter or spring quarter.
